Jolie proves that she's more than just a pretty face

One of the world's most famous actresses came to Washington last week to promote her latest movie. But in more than half an hour of conversation, Angelina Jolie didn't once touch on any tabloid topics.

One word repeatedly came up: "intervention."

The actress wasn't talking about getting a troubled colleague into rehab. She meant it in the political sense, and she repeated it because she believes the United States, and the international community, should be intervening sooner -- and more often -- in ugly conflicts around the world.I won't deny it: It felt surreal to be having a political discussion with the woman People magazine named the world's most beautiful in 2006. It was also surprising. As a film critic and writer in the capital of the free world for years, I've heard countless celebrities make political pronouncements. Almost none of them sounded as thoughtful -- and as nuanced -- as Jolie.

One expects to meet the 36-year-old Jolie and find she's as elegantly attractive in person as she is on screen. One doesn't expect to discover she seems more keenly intelligent than most professional pundits.

Jolie came to the District to talk about her latest film, in which she actually doesn't appear. "In the Land of Blood and Honey" is her debut as a writer-director. And an unlikely debut it is: The film is almost entirely in the Serbo-Croatian language. It tells the tale of two citizens of Sarajevo during the bloody Bosnian war of the 1990s: a Serb army officer and a Muslim painter, who dated before the war and find themselves on the opposite sides of a prison camp after it begins.

"Blood and Honey" is, more than anything else, a war film, though unlike most in the genre, it focuses on what happens outside the combat zone. It's a particular story of two (imagined) people, but Jolie is quick to admit she made it with a larger point in mind.

"What a horrible thing we did internationally not to help these people in a timely fashion," she says. "All the issues in this film are going on right now in many other places in the world."

With that move -- from the world and time of her film, to the present day -- Jolie makes a definite statement in the part of the political arena in which she's most interested: foreign policy.

Jolie became a member of the Council on Foreign Relations in 2007 and a Goodwill Ambassador for the United Nations High Commissioner for Refugees in 2001.

"We need to be doing more," she flatly declares of American work in the region.

She went to Syria in 2009 to meet Iraqi refugees, along with Syrian President Bashar al-Assad and his wife, Asma. Two years later, she traveled to Turkey, to talk to refugees there from the violence in Syria, where Assad's response to protests has been murderous. She plans to meet with many more.

I ask about regime change in Syria; Angelina Jolie warns that we don't want the Arab nation to become a satellite of its Persian neighbor, Iran.

This is unlike any celebrity interview, ever. Those who worked with her on "Blood and Honey" were just as surprised.

"The first time we met, she knew more about the history of my country than I did," reports star Zana Marjanovic. "It's so important, because art is a serious thing."

"Blood and Honey" won't do as big business as films starring Jolie end to do. But that won't stop her from making more movies that, in some way, make political arguments. She says she's already written her next project -- on Afghanistan.

Angelina Jolie Spends Time in Afghanistan With Refugees

If you were like us TGB'ers, you noticed that Angelina Jolie did not grace us with her beauty on the red carpet of the Oscars. It was reported that instead of spending the time all dolled up in Hollywood, instead the Jolie-Pitt family were in Italy with the kids.

Perhaps Angelina was also getting ready for her recent humanitarian trip to Afghanistan. The actress turned goodwill ambassador for the United Nations High Commissions for Refugees, met with refugees returnees in Kabul, many of whom had been exiled. Over 5.5 million refugees have returned to the country since 2002, many from Pakistan and Iran, and now make up 20% of the population.

"It’s clear travelling through the country that what needs to be done is a very focused approach in places of return," Jolie said. "We need to revisit the idea of what return is and the difference between just returning and reintegrating.

"The focus needs to be put now on reintegration, and that means not just putting up shelter but making sure there is water, job opportunities, a school for the children and medical clinics."

She also met up with Khanum Gul in the small room she shares with her husband and eight children. During Jolie's first time to Afghanistan in 2008, she met with Gul, who had just given birth to her eighth child, a son named Samir.

"It was very distressing to see that, because of the poor conditions, Samir seems to be suffering some form of developmental delay due to malnutrition or lack of medical care. He can't walk and is barely surviving in what can only be described as a very cold and damp warehouse," Jolie said.

Gul also supports her ailing mother, and her husband, a day laborer, is having a difficult time finding work.

"This old woman was so upset, because she feels like a burden. She watches her grandchildren go onto the streets every day to wash cars for a dollar a day so the family can eat. Often they earn nothing," Jolie said. "Everyone I have met on this visit has been very clear. The Afghan people don't want to become beggars. They want the opportunity to work for a living with dignity so they can provide for their families."

In addition to her work with the refugees in Kabul, Jolie traveled to Qala, north of Kabul, at met with young girls who will be studying in a new primary school being built in the village, a school the actress is funding.

The two begin a romance but Frank soon finds himself in a thrilling and dangerous web of intrigue as his new lover is not all that she appears.Directed by Florian Henckel von Donnersmarck, the movie nearly failed to get off the ground after some cast and crew issues, in which Avatar star Sam Worthington was initially pitched as Johnny Depp's character. However, after 'creative differences' both lead Worthington and director von Donnersmarck walked away from the project until Depp replaced the Avatar star and von Donnersmarck made a return.

The movie is billed as ultimately a love story, but it is also a comedy thriller that is sure to attract both Jolie and Depp fans alike. 'The Tourist' is set for release 10 December in the USA.

Angelina Jolie visited earthquake victims in Haiti on Tuesday in her latest trip to help survivors of conflict and natural disaster as a goodwill envoy for a United Nations agency.

Jolie, 34, who has been a goodwill ambassador for the U.N. refugee agency UNHCR since early 2001, traveled to quake-shattered Port-au-Prince after visiting injured Haitian children in a hospital on Monday in neighboring Dominican Republic.

The actress, who has been on numerous U.N. humanitarian missions to help refugees from more than 20 countries, was due to visit a hospital and other relief sites treating hundreds of thousands of hurt and homeless Haitian quake survivors.

The dark-haired Hollywood star, wearing a black jacket and aviator sunglasses, waved to reporters when she visited a U.N. base near Port-au-Prince airport, before she was driven away in a white U.N. vehicle.UNHCR officials in Port-au-Prince told reporters her visit was "private."

In Geneva, UNHCR spokesman Andrej Mahecic declined to give specifics about Jolie's itinerary in Haiti, citing security reasons for its long-standing policy of not disclosing details of trips by its goodwill ambassadors.

On Monday in the Dominican Republic, the actress visited a hospital in Santo Domingo where dozens of injured Haitians, mainly children, have been treated since the devastating January 12 quake in Haiti that killed more than 200,000 people.

Angelina Jolie and Brad Pitt are suing the News Of The World newspaper after it published allegations about their relationship.

In a story published on 24 January, the paper said the couple would separate and had agreed the division of assets and custody of their children.

The couple's lawyer, Keith Schilling, called the "widely republished" allegations "false and intrusive".

He added the paper had failed to meet "reasonable demands" for an apology.

He said the pair had also asked for a retraction of the allegations, which had subsequently been "widely republished by mainstream news outlets". Not married

A statement from Schillings Lawyers also noted that Sorrell Trope, identified by some publications as a divorce lawyer advising Pitt and Jolie, had never met them.

"I have had no contact from... Angelina Jolie and / or Brad Pitt," said Trope in a letter quoted in the statement.

"I have never met... your clients or had any involvement with either of them. The foregoing is true with respect to all other members of this firm."

The action comes following the publication by the News of the World of allegations that Jolie and / or Pitt had visited a divorce lawyer in December, and that an agreement had been reached between them regarding custody of their children and the dividing of their joint assets of £205m.

The newspaper added that the agreement had been signed last month and that the separation would occur imminently.

A spokesperson for the News Of The World was unable to comment at the time of writing.

The Risk of a Stroke During Plastic Surgery

While most plastic surgeons will outline the risks of having an elective surgical procedure performed, very few tend to speak directly to the issues regarding safety and post operative complications. Often, a surgeon will make a general statement regarding the risks of surgery, such as, "All surgery carries risks which includes, bleeding, infection, difficulty breathing, swelling, clotting, and other illnesses in a small percentage of patients," and then move directly into talking about the benefits of the procedure. Patients need to inform themselves of the actual risks associated with cosmetic surgical procedures, especially the risk of a stroke or cerebral vascular accident (CVA). As a physician, I understand that there is no value in frightening our patients without cause. Yet I still firmly believe that patients should never be too quick to decide on an elective surgical procedure without fully realizing the potential for devastating complications.

Patients and physicians alike need to recognize that most patients who are receiving some sort of cosmetic care are generally considered to be healthy individuals. Thus, neither patient nor physician is necessarily ready for the onset of a complication. This fact alone can make a complication much more serious than if the very same complication arose during a medically necessary procedure. For instance, even during a routine knee surgery, physicians do not see their patients as completely healthy, and are more guarded against the onset of blood clots or other risk factors for stroke.

Who is at Risk?

Few patients really consider themselves at risk for a possible stroke because they associate the condition as an event that occurs to people over the age of 55. While in some cases age does bear a factor, it is far from the determining factor of a stroke. In fact, a healthy 65-year-old woman can be less likely to have a stroke than a 35-year-old woman who is undergoing invasive cosmetic surgery. Patients should understand the causes of a stroke in a post operative environment in order to assess more realistically their level of risk. However, as a physician I can testify to the fact that sometimes there simply is no way to ascertain why a stroke occurred for one individual and not for another with the same health history, the same risk factors, and the same surgical procedure.

Most post operative strokes occur when a blood clot dislodges, which typically occurs in the lower section of the body. This blood clot then travels through the body and may lodge itself in the brain or more commonly the lungs. This action of a blood clot traveling through the body post surgically is commonly referred to as an embolism and usually arises from a larger blood clot that usually forms in the lower body known as a deep venous thrombosis or "DVT". DVT is a prime concern for surgeons, and can be considered a precursor to a stroke.

Understanding the Potential Danger

Blood clots generally become trapped in either the lung or the brain when they break free from other parts of the body. When the blood clot becomes caught in a blood vessel in the brain, it results in a CVA or stroke. When it becomes trapped in a blood vessel in the lung it is known as a pulmonary embolism or PE. Either condition is life threatening and can carry serious consequences.

While blood clots and strokes are a significant risk for all patients having any type of invasive surgery, plastic surgeons run into a unique scenario that generally makes it a little more difficult for our patients to get treatment and help in a short amount of time. Incidents of blood clots and stroke can happen anywhere between several hours post operatively up to about ten days after surgery. Thus, these incidents rarely occur within a medical environment and usually happen at home. Patients who remain in the hospital or a medical center after a medically necessary procedure are more likely to have these incidents in the hospital.

Patients, who are going to experience a cosmetic procedure, or any type of surgery for that matter, should familiarize themselves with the general symptoms of post surgical pulmonary embolism or stroke. Symptoms that include shortness of breath, chest pain, disorientation, and sometimes even a sense of altered mental status are likely either to be related to a pulmonary embolism or a stroke. Patients and physicians alike should also understand the basic risk factors for these complications and prior to a surgical procedure. While there are still studies being conducted, which will help determine a patient's risk level in the future, there are some pretty basic factors, which point to a likely candidate.

Risk Factors For Patients

Women are more likely to experience these types of complications than men, as are smokers. There are numerous medications, which place patients at a higher risk for postoperative blood clots, including but not limited to hormone replacements and birth control pills. Weight plays a significant factor as does the level of activity the patient generally participates in. Patients who generally lead a sedentary lifestyle are at a higher risk. Obese patients are at an even higher risk. Increased age and a previous history of deep venous thrombosis are more obvious risk factors. Patients who have a current diagnosis of cancer are at a particularly high risk.

Reducing the Risk Factors for Safer Surgery

Whenever a patient is booked for surgery in my practice, I always insist that they have a medical clearance by an internist to help determine their general risk level for elective surgery and the possibility of medical complications, which includes the risk of abnormal blood clotting. I also take into consideration the type of surgery that I am performing on the patient. In general, surgical procedures that have a longer operative time and involve body contouring carry a higher risk than those that are minimally invasive and are performed on the head and neck. The internist will review a list of current and recent medications, as I do myself, and will verify that the patient is physically fit to tolerate the procedure that is desired. There are still no guarantees, but knowing a patient's medical history, their obvious risk factors, and having a second pair of eyes look over the same information that I am presented with, ensures that I am taking all possible precautions.

I also use something known as pneumatic compression boots for every patient undergoing any surgical procedure and I urge other physicians to do so as well. These boots are able to keep the muscles of the lower extremities moving in a fashion similar to walking, which can help prevent blood clots. I insist that my patients carry on an active lifestyle, to help prevent DVT formation and to directly assist with both preoperative and postoperative care. Proper positioning in the operating room allows for additional prevention of blood clot formation as well.

Physicians and patients need to understand that the potential for serious complications related to cosmetic surgery can be devastating. Knowledge is power and education can help reduce the number of blood clots and such rare but devastating complications as strokes that occur after surgery. With current research and education practices, we should be able to reduce the number of post operative strokes and blood clots over the coming years.
